Job Description: The LPN participates in the day-to-day coordination and delivery of skilled nursing services and performs related divisional teaching/visit duties to ensure the highest quality of care. Ensures that delivery of nursing services is compliant with company/state policies. Available resource person to other health team members in the identification and fulfillment of patient needs. Maintains liaison with the community’s health-related resources. Qualifications, Knowledge, Skills, and Abilities: Graduate of approved School of Professional Nursing - or - Current state Licensed Practical Nurse license. Pediatric nursing experience preferred.
